Lab Slip & Prescription Guide

A clear lab slip gives Damore the product selection, clinical instructions, and office information that belong with the case. Use the current form and make the special instructions specific enough to explain what your office is requesting.

Put the important details where the lab can see them.

Office and case information

Identify the practice, the prescription, and the return details

Provide the office name, prescribing dentist, office contact details, and the case information requested on the current form. Include the date sent, date needed, and authorization details.

Keep patient-identifying information in the approved case-submission process. The general contact form is not a replacement for a lab prescription.

Crown and bridge details

Make the restoration and design clear

Identify the teeth involved and the requested material or product. Where relevant, include shade and staining instructions, contact preferences, pontic design, margin details, anterior design, and occlusion.

The special-instructions area is the place to explain a detail that is not captured by the standard choices. Contact the lab when the prescription needs a technical discussion.

Denture and partial details

Identify the appliance and the stage of work

Specify upper or lower work, the requested denture or partial type, and the prescribed stage. Include shade, mould, relevant measurements, framework or clasp instructions, and any other case details.

For a name-identification request, provide the exact spelling through the case-submission process. For a repair or tooth addition, describe the existing appliance and the work requested.
